  Technical Difficulty  
     
 
1)   This site is optimized for 1024 X 768 screen resolution and is best viewed with IE 7.0 with pop-up blocker is disabled.
 
 
2)   Adobe Flash player. The recommended version is 9 or above.
 
3)   Java Runtime Environment. The recommended Java version is 1.5.0 or above.
 
4)   All downloadable forms are saved in pdf form which requires Adobe Acrobat Reader. This program is necessary if you want to view any of our downloadable forms over the internet. Please ensure that you have the Adobe Acrobat Reader downloaded.
 
 
5)   You may sometimes experience short delays for the pages to load due to the following reasons.
   
 
  1. The speed of your computer's processor.
  2. The speed of your modem or network connection.
  3. During peak hours, Internet can sometimes experience traffic jams.
  4. The amount of memory allocated to your browser. Closing some programs may increase performance.
  5. Large images contribute to page loading time. We have taken care to limit the number and size of images to help the pages load more quickly.
Please be patient and do not hit the Stop or Reload button. Do not resubmit the form. If the problem persists, please contact ROM to report it so that we can investigate the matter further. It will be helpful if you can describe as clearly as possible the problem you are having, browser version, problem on the service name, description of problem, the exact error message or screen dumps, the steps you take which lead to its occurrence and the exact text of any error messages you receive.
 
     
     
  Printing Instruction  
     
  Please choose 'Landscape' mode instead of 'Portrait'. This is to ensure information on the right side of the page is not cut off. Please follow these steps before printing:  
     
 
  1. Select 'Print...' from the 'File' menu
  2. From the 'Print...' dialog box select either 'Properties' or 'Page setup...'
  3. In the orientation section of the 'Page setup...' dialog box, select 'Landscape' mode
 
  You may use the 'Print Preview' function to view your printout before printing to check for the margins. If you encounter any problem, consult Help within your browser.
